USASA INCIDENT REPORT REVIEW

You can submit an injury claim If you are a registered USASA player and your injury occurred during a sanctioned USASA event. Players must file their claim within 90 days from the date of the injury.

When completing the claim, please make sure your contact information, including date of birth and injury date, is correct. After your claim is approved, you will receive an email and an acknowledgment letter by mail with further instructions.
